facilities to provide your care. You can use this site to find preferred providers and
facilities.
Since Significa uses multiple networks of health care providers, please look at your Significa
identification card to select preferred providers.
Significa EHP PPO
State-wide, the EHP Significa or Significa PPO network is available to certain employer groups.
If the EHP Significa logo appears on your card, then you have access to this network. To find

Significa Susquehanna PPO
In North Central PA, you will receive quality health care from a strong local network of more
than 3400 participating physicians in 11 counties, with access to an exceptional network of
hospitals including those in the Susquehanna Health and the Penn State Milton S. Hershey Medical
Center.

Significa Traditional PPO
In Northwestern PA, access the area’s most highly respected providers through Significa’s
Traditional PPO network with Vantage PPO’s high-quality network of hospitals, doctors and allied
health facilities. Your ID card shows the Significa Traditional network with the Vantage PPO logo.
How To Select a Tier 1 Hospital
You will receive the highest level of benefits by seeking treatment at a Tier 1 hospital in
Northwestern PA. To find a Tier 1 facility, select the Significa Traditional PPO network and enter
one of these search criteria: Name, Specialty or County. Your search results will state whether
the hospital is 'Tier 1.'

OUT OF PA – MultiPLAN / PHCS
If you need to access care outside of PA, such as in an emergency or if you have out-of-state
dependents, use this link to select a doctor or provider from the MultiPlan / PHCS network.
