Significa's Three-Tier Product Options

Significa Three-Tier plans are only offered in northwestern Pennsylvania and provide three tiers of benefits, each with separate coinsurance, deductible and maximum amounts.

Tier 1: Combines the highest level of benefits with a variety of excellent regional hospitals, including many Vantage facilities;
Tier 2: Provides a traditional level of benefits through all Vantage doctors, additional Vantage facilities and many UPMC
             hospitals;
Tier 3: The lowest level of benefits when out-of-network providers are used.
