Significa ensures that you have access to several doctors, health care providers and facilities to provide your care. You can use this site to find preferred providers and facilities.
Since Significa uses two different networks of health care providers, please look at your Significa identification card to determine the network from which to select providers.
Select a Doctor
If your ID card states to use the AFMC network, use this link to select a doctor/provider from the AFMC network.
Once the page opens, please choose from these categories in the drop down menu:
- Medical
- Allied Health
- Behavioral Health
- Chiropractic
Select a Facility
Your ID card will state if your coverage is through our PPO or POS plan.
To select a facility from the AFMC network, select the plan type from the drop down menu that corresponds with your plan type, as listed on your ID card:
(If you have POS coverage, please note that facilities with the note “POS1” provide you with the greatest out of pocket savings.)

If your ID card states to use the MultiPlan / PHCS network, use this link to select a doctor/provider from the MultiPlan / PHCS network.
Once the page opens, please select the MultiPlan logo and/or the PHCS logo to find a participating doctor or facility from the MultiPlan / PHCS network.

If your ID card states to use the HMA, Inc. (AZ EPN) network in Tucson, use this link to select a network doctor or provider in the benefit tier for the coverage you want:
- Tier 1 (EPN) or
- Tier 2 (PPO).
Doctors and providers in the Tier 1 (EPN) network provide you with the greatest out of pocket savings.
