Chronic Obstructive Pulmonary Program

Significa offers a program to help you manage Chronic Obstructive Pulmonary Disease
(COPD), a lung disease that affects millions of people. COPD includes diseases such
as emphysema and chronic bronchitis. About 24 million adults in the U.S. have evidence
of impaired lung function indicating that COPD is under-diagnosed. If you have COPD,
it is important for you to learn about your disease and how you can manage it.

A person with COPD can have either emphysema or chronic bronchitis, but most have
both. Some people with COPD may also have "asthma-like" symptoms, such as wheezing.

The Depression Management Program offers a supportive service that helps you to effectively manage depression at a time when you may need it most. You may be struggling with not only the symptoms of depression but possibly difficult personal or situational circumstances as well. This program aims at ensuring continuity of care for those enrolled with their doctors by providing educational materials and self-management tools; regular phone support from a Care Coordinator and encouragement to follow treatment plans; and referral resources to behavioral health care services.

Prenatal Program

If you are expecting, the Prenatal Program can support you through your pregnancy
to help you approach pregnancy with confidence. Pregnant insureds are initially sent a health assessment form. Information packets relevant to each of the three trimesters of pregnancy are sent once the completed form is received. A Prenatal Program Care Coordinator initiates phone calls to assess pregnancy progress, address concerns and answer any questions.
