Personal Health Record

In order to be eligible to utilize this exciting new application, a Personal Health Record (PHR) must be created via our health portal, MyHealth Portal. You can access this application by clicking here.

Status Pending

Upon completion of the PHR form, you will not have automatic access to the AllOne Mobile application. Acceptance into the system can take up to 24 hours. During this time, you can access and modify any of your PHR data via the MyHealth Portal by logging in anytime.

Step #3
Mobile Device Activation

The final step required for the activation of AllOne Mobile is to determine the device your mobile application will operate on. When you first enter the AllOne Mobile portal, you will be prompted to enter the information for your mobile device. The items that will be collected are as follows: phone number, manufacturer, model, and carrier.

Once this information has been successfully entered, you will be sent a text message containing a link to download the appropriate application to your mobile device.  At this time you will also have access to all the features located inside the AllOne Mobile web portal.
